PATIENT INFORMATION

First Visit

Thank you for considering us for your dental needs.  We take immense pride in treating every patient as if they are family and personalizing treatment to fit each patient's needs, both dentally and financially.  We believe that no two cases are the same and we strive to provide the highest quality of care for each individual patient.
 
For all new patients, we recommend setting up an appointment for a comprehensive exam with the doctor prior to scheduling your cleaning.  This will allow you to get a feel for our office, meet the doctor, and get all your questions answered.  Also, as all patients have different needs when it comes to getting their teeth cleaned, it will allow us to schedule you appropriately with one of our hygienists.  
 
If it is determined that you have dental needs other than just a simple cleaning, we will set up an appointment accordingly. Also if you do not need a cleaning at this time but have other dental needs, we would be happy to set up a consultation with  Dr. Edward Nelson or Dr. Andrew Nelson.  Please inform our office prior to your first visit if any precautions must be taken for dental treatment* (see below).   
 
We also understand that Cape Cod is a popular vacation destination and a dental emergency may often occur during your day at the beach, and a trip home to your regular dentist is not possible.  If this is the case, we will gladly coordinate with your regular dental office to take care of your dental emergency as soon as possible. 
 
Please assist us by providing the following information at the time of your first visit:
  • Any X-rays taken at your previous dental office (bring a copy or email them to us at [email protected])
  • A list of medical conditions and any medications you are presently taking.
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • Please take the time to complete and submit the New Patient Questionaires under the patient login section (you will need the date of your appointment to complete this)
  • You may also register yourself as a patient under the patient login section, which will allow you round–the–clock access to view upcoming appointments, treatment plans and account balances. You can even request appointments and make online payments!

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
